Please provide as much information as possible.  A member of our Family Connects NJ team will follow up with some additional questions and to schedule your in-home nursing visit.


If you have needs while you are awaiting your nurse visit, you can also connect to services through your county-based Connecting NJ hub. Connecting NJ provides information and referrals for both health and social resources. They can also screen families for eligibility for other home visiting programs available in your county. As part of Family Connects NJ, we have community alignment specialists stationed at each Connecting NJ hub to help develop resources for expecting families and families with newborns.
