Family Connects NJ is changing the way postpartum care is delivered to families across the state. Every family caring for a newborn, or recovering from a birth can benefit from a nurse visit at home. To that end, Family Connects NJ nurse visits will be rolled out incrementally until they are available to families in all New Jersey counties.

Beginning in January 2024, the program will be available in Essex, Middlesex, Mercer, Cumberland, and Gloucester Counties, and will expand to additional counties on an annual basis.

The anticipated schedule for expansion is as follows:

  • Phase 1 (January 2024): Family Connects NJ Services offered in first 5 counties (Essex, Middlesex, Mercer, Cumberland and Gloucester)
  • Phase 2 (January 2025): Family Connects NJ Services available in previous counties, plus Somerset, Sussex, Passaic, Hudson, Bergen, and Ocean Counties.
  • Phase 3 (January 2026): Family Connects NJ available in all previous counties, plus Camden, Salem, Atlantic, Burlington, Cape May and Monmouth Counties.
  • Full Statewide Implementation (January 2027): The final counties of Union, Hunterdon, Morris and Warren launch their services. Family Connects NJ will now be available everywhere in the state!

Family Connects NJ Community Alignment Specialists are stationed in each county's Connecting NJ hub. These Specialists develop Family Connects NJ community events designed especially for families expecting or raising young children. They also assist the Connecting NJ staff and Family Connects NJ nurses by identifying needed resources available in their community and developing partnerships with community organizations serving local families. These resources are available in all counties, even if nurse visits have not yet started.
