Family Connects NJ nurse home visits will be available in your county soon! Beginning in the new year, you will be able to sign up to have a nurse visit your family, answer your questions, and ensure the birthing parent and newborn are both healthy--all in the comfort of your own home!

To receive updates on when you will be able to sign up for your own nurse home visit through Family Connects NJ, please complete the form below. You will receive a message when the online booking tool is live for your county, in early January 2026.

If you need immediate assistance for your newborn, you may be able to connect to help through one of New Jersey's county-based Connecting NJ hubs, which are currently active in all counties.
